Request a Certificate, Proclamation or AwardCertificates and AwardsCertificates are issued by the Office of the Governor to recognize individuals for their contributions to the State of North Carolina. The goal is to honor individuals who have made a significant impact towards creating a better North Carolina.
To request a certificate complete a certificate request form and email it to certificates@nc.gov or mail to: Governor's Office of Constituent Services Please Note: Unlesss certificate requests are submitted 6 weeks prior to the date needed, we cannot guarantee receipt as requested. Sufficient time for the approval process is imperative. Click here to see descriptions of available certificates. LettersLetters of recognition are issued by the Office of the Governor to recognize special occasions for individuals such as Anniversaries, Birthdays and Retirements. To request a letter, please submit a letter request form and email it to letter.requests@nc.gov or mail to:

ProclamationsProclamations are issued by the Office of the Governor to recognize special events or significant issues. The goal of a proclamation is to honor and celebrate events or to increase awareness of significant issues of importance to North Carolinians.
To request a proclamation complete a proclamation request form. Submit the form and draft language for the proclamation to proclamations@nc.gov or mail to: Governor's Office of Constituent Services Requests should be submitted 6-8 weeks in advance. Requests will be considered by the Proclamations Committee, and you will be notified of a decision in a timely manner. |
